Please advise us if in the 14 days prior to your scheduled appointment you have:

1. Travelled outside the province
2. Have been in contact with someone who has been diagnosed with Covid 19
3. Are experiencing symptoms of COVID-19 defined as having two or more of the following (new or worsening): Fever (or signs of a fever, including chills, sweats, muscle aches, lightheadedness) cough, headache, sore throat, painful swallowing, runny nose
diarrhea, loss of sense of smell or taste, or unexplained loss of appetite.
